Student Medical Examination Report

The Student Medical Examination Report is a health form that new students of Vavuniya Campus must complete before entering the university. The information is confidential and is used only by the University Health Service.

How to complete the form

  • Part I is completed by the student. It covers personal details, family medical history, the student’s own medical history, and immunization records.
  • Part II is completed, signed, and stamped by an MBBS-qualified government medical officer.

Send the completed form to the Medical Officer, Assistant Registrar, Student & Welfare Division, Vavuniya Campus, Pambaimadu, Vavuniya. Please attach a copy of your Child Health Record.
