Notice: Name Confirmation for Transcript and Degree Certificate

This Name Confirmation for Transcript and Degree Certificate notice requests all Faculty of Applied Science students to confirm the spelling and spacing of their names as listed in the annexed table below. The Examination Branch, University of Vavuniya, will use the confirmed name for the Transcript, Statement of Results, and Degree Certificate.

How to Request a Correction

Students who notice an error must inform the Examination Branch through the Faculty of Applied Science AR office on or before 04.09.2026, by submitting:

• A request letter addressed to the Examination Branch
• An affidavit confirming the correct full name in English

Students should hand over these documents to the AR office at the Faculty of Applied Science. Please note that the university will not consider requests submitted after the deadline under any circumstances.
